Do you give permission to Brittany Lewis and Uncommon Sense Therapy to use pictures, videos and materials from the Pilot Program you're in to promote the business/program in the future through advertisements, social media and online?

Do you understand that by giving this permission you forfeit all rights to the graphics, images and videos all all proceeds generated from them will go to Brittany Lewis and the proliferation of Uncommon Sense Therapy?

Do you understand that by selecting "No" in either field the agreed upon exchange will no longer be valid and you will be removed from the program?
